These are certain items/modes that you'll be able to unlock and use/play when completing Metal Gear Solid and doing other things.
 Alternate Costume for Meryl
Complete the game while wearing the tux and get Meryl's ending.
 Alternate Costume for Otacon
Complete the game while wearing the tux and get Otacon's ending.
If you complete the game with Meryl's ending (not submitting to the torture), you'll receive this item. It will give you infinite ammo on all your guns.
 Boss Survival Mode
Complete the game once with either ending.
Once you complete the game twice, you'll unlock the camera. Not really useful, just for taking pictures of things throughout the game.
 Demo Theatre Mode
Complete the game with either ending; complete both endings to completely unlock this option.
 Different Ending Music
Complete the game while wearing the tux.
 Extreme Difficulty Mode
Complete the game on any difficulty.
 Meryl Save Icon
Complete the game with Meryl's ending, and then save.
 MGS Theme during End Credits
Complete the game three times on the same save file.
 Ninja Save Icon
Complete the game twice, and then save.
 Otacon Save Icon
Complete the game once, and then save.
 Stealth Camouflage
If you complete the game with Otacon's ending (submitting to the torture), you'll receive this item. It will make you invisible from all enemies, except the bosses, and scripted alerts where fighting with enemy soldiers is unavoidable (the communications tower run, the held elevator, etc).
Complete the game twice, once with Meryl's ending and once with Otacon's ending. Save them on the same memory slot. Then, choose New Game, Snake will change into the tux when going up the elevator.
